What is the Questionnaire Tab?


The Questionnaire Tab is a section within the Referral Details screen that allows you to view and input details about the person as they relate to the service being requested through the referral. Each service may have it's own set of information defined in the Questionnaire; some required, some not. These configurations were defined through the course of onboarding your organization into the system.

At the top of the tab, you will see a menu of categories the Questionnaire is broken down into. Click on any of these sections to view the set of data points/questions associated with the categories.

  1. Any data point that has a finite set of pre-defined entries have the ability to be flagged for Consideration. These data points and their values and behaviors are also defined through the course of onboarding. Any entries that meet criteria for consideration will be flagged as such and be compiled in the Referral Summary Consideration.
  2. Any data point that is defined at required will be indicated with a red asterisk (*).
    In the case of the Questionnaire, Supportable does not enforce requirements in any way, but rather communicates and displays the status of any requirements through the Missing Details section of the Referral Summary. This is because the data points of a Questionnaire in many cases or services might be gathered throughout the intake process. So, if required pieces of the Questionnaire are unsatisfied, they will be displayed as such in the Referral Summary, allowing anyone with the delegated responsibility to review this at any time. 

    It is recommended that a specific role is created to delegate the responsibility of Queue Approval, allowing for a natural process flow to a person to inspect the referral before approving it into the Queue and scheduling Admission.
  3. Any time all required data points within a Category are satisfied, the Category label will turn green.
