Referral Cover Page

What is a Referral Cover Page?

A Referral Cover Page is an overview of the status of a given referral. Upon opening a referral, a right-drawer will open, providing you the latest and most need-to-know information all summarized in one screen; prompting you to take action based on your role.

Bottom Navigation

The bottom of the Referral Cover Page will always offer you a selection of buttons based on the referral's status and the User's visibility. These buttons become your primary way to navigate and process a referral.

Reject: Opens the window to select reasons for rejecting a referral and enter a description.
View Referral Details: Opens the Referral Details screen where you are able to see the most granular information about a referral. It contacts an array of tabs that allow you to get work done and document information gathered.
Close: Takes you back to the Intake Overview screen.
Bypass Stage: From time to time, there may be an exception to the rules of a Stage. For example, a referral might not require an assessment because the person has already been assessed by another agency prior to the referral. Any special cases can be documented in internal Notes and the referral can skip the stage.
Next Stage: When all Stage-specific criteria and Checklist items have been met, you can advance the person to the next stage. This button will not appear until the referral has met criteria.

Intake Progress Bar

The Intake Progress Bar provides you a summary of a referral in the current state it is in, leveraging on 2 criteria:

  1. The left half of the circle, as well as the color of the circle, is designated to the Stage-specific criteria (Unscheduled, Scheduled, Past Due, etc.). This half of the circle will fill when criteria is met.
  2. The right half of the circle is designated to the stage's Checklist. When all Checklist items are met, this half of the circle will fill.
Only stages that apply to the respective service at hand will appear in the progress bar.
Pro Tip: You can hover over a completed stage to see a tooltip that will give you information about that stage being completed.

Show Latest Activity

Beneath the Intake Progress Bar, you can click the Show latest activity link to see highly detailed information about the history of the referral. Certain items on this history, depending on the nature of the item, are able to be clicked into to view and edit the actual feature related to the item. All items can be hovered over to view even more granular information.

Consideration

The Consideration section summarizes information from the Questionnaire for you as it is being filled out per your organization's tailored setup. The areas you've defined as worth considering based on an entry will then float to the top of the Referral Cover Page, allowing you to easy review as you continue to advance a person through the Intake Process.
Information in the Consideration section is Read Only. Clicking the down arrow will collapse the section if you so choose.

Missing Details

The Missing Details section summarizes the information that is missing from the person's record or the Questionnaire.
As information for the person's record and the Questionnaire are gathered entered, these items will disappear from this section.

Stage-specific Progress

This section appears after the screening stage, allowing you to enter information, mostly dates, on activity that happened during that stage.
Each stage has it's own requirements for completion.

Checklists

The Checklist section keeps all the granular or outside-of-Supportable tasks in one place. As we know, each service may have specific things that might need to be done in any given stage. For this reason, there is a checklist for each service for each stage, all of which are able to be tailored by your team.

With the exception of the New Stage, which does not have a Checklist, the completion of a Checklist is a requirement before advancing a referral to the next stage.

Checklists are able to be modified by an Admin User through the ticketing system.

When freshly opening a referral in the Referral Cover Page, the view will default to display the Checklist for the current stage. Any Checklists for future stages have not yet been generated. Any completed Checklists for previous stages are able to be viewed by clicking the link of the respective checklist to expand it.

Checklist Items

There are two types of checklist items:

  1. Items that may not be applicable to a given referral, and therefore offer an "N/A" option. When "N/A" is selected, a note is required.
  2. Items that do not have an exception and simply have a "Done" option. Notes are optional for these items.
To complete a checklist item, simply select the option that applies, make a note if appropriate, and click the Save button.

For Users with the permission defined their role, Checklist Items can be edited if necessary. Be aware that this action will overwrite the previous note. If you prefer to preserve the original note, simply add any comments you may see necessary after the original note.
